Forms Effective on December 1, 2026
Amended Official Forms 101 and 101C were approved by the Judicial Conference at its September 2026 meeting:
- Official Form 101 is amended to remind debtors and counsel to not list employer EIN numbers on the form in lieu of a Social Security Number.

Official Form 106C is amended to provide a total of the specific dollar amount of the exemptions and adds a space on the form for the debtor's total interest in the exempt property.
